Thinking of Becoming a Wellness Member???

Three Ways to Join the Wellness Program:

1) Request an Application by contacting Sharon Sathuthiti by phone (718) 409-9444 ext. 17 or email ssathuthiti@wspt.org . Fill the Wellness Program Application out completely and return it via mail to WSPT 1250 Waters Place, Suite 501 Bronx, NY 10461 or fax it to us at 718.409.6539

2) Print off the Wellness Program Application , fill it out completely and return it via mail to WSPT 1250 Waters Place, Suite 501 Bronx, NY 10461 or fax it to us at 718.409.6539

3) Stop by in person to request an application, fill it out, and return it on the spot or take it with you to fill out and return later.
